sshd-session likely remains stuck in an unclassified userspace loop. A diagnosis report and external handoff were created locally.

Likely owner

openssh

Reason: workspace-acquisition

Suggested bug target: https://www.openssh.com/

Next steps

  • File an upstream or vendor issue with the diagnosis bundle and package metadata.
  • Include the workspace acquisition note so maintainers know why no local source patch was attempted.

Technical snapshot

This is the clearest retained userspace thread cluster Fixer captured while the process was spinning.

Representative thread backtrace

  • Command: sshd-session: gis [priv]
  • Why Fixer classified it this way: The process is demonstrably CPU-hot, but the current syscall and symbol sample does not point to a single dominant loop family yet.
  • Thread summary: thread backtraces show 1 thread(s) around 0x00007f70c809a7d2 in ?? () from target:<path>
  • Contention signals: event-loop-wakeups, gdb-stderr: gdb: warning: Couldn't determine a path for the index cache directory.
  • Repeated loop: restart_syscall
  • Top syscalls: restart_syscall x1
  • Package: openssh-server 1:10.3p1-1

Collected context

  • Subsystem: runaway-process
  • Diagnosis family: unknown-userspace-loop
  • Hot path: do_sys_poll
  • Command: sshd-session: gis [priv]
  • Thread backtrace summary: thread backtraces show 1 thread(s) around 0x00007f70c809a7d2 in ?? () from target:<path>
  • Source package: openssh
  • Workspace source: debian-source
  • Workspace acquisition: Fetched Debian source package via apt-get source.
  • Homepage: https://www.openssh.com/
  • Implicated packages: libc6, linux-image-6.17.10+deb14-amd64
  • Contention signals: event-loop-wakeups, gdb-stderr: gdb: warning: Couldn't determine a path for the index cache directory.

The process is demonstrably CPU-hot, but the current syscall and symbol sample does not point to a single dominant loop family yet.

## Plan Pass

You are planning a fixer patch before any edits happen.

Read the evidence bundle at `./evidence.json`. The prepared workspace is `./workspace` and it was acquired via `debian-source`. The original pre-edit snapshot is available at `./source` if you need to inspect it. For interpreter processes, plan from the script/application entrypoint evidence first and include the runtime only as a second investigation target unless the evidence proves a runtime bug.

Validation expectation: try the project-level build/test entrypoint from the workspace root before reporting only a focused leaf compile. Detected candidate(s): `./configure && make`. If the project-level command fails because dependencies or generated files are missing, include the exact command and failure reason in `## Validation`, then run the narrowest relevant compile/test that is still reproducible from a clean checkout.

Upstream-style expectation: before planning or editing, check for contribution/style docs (`CONTRIBUTING`, `HACKING`, `README-hacking`, `README.md`, `docs/`, `dev-docs/`) and scan the touched subsystem for local helpers. If the project has wrappers for file IO, path-relative IO, process spawning, memory allocation, logging, locking, or platform compatibility, prefer those wrappers over generic libc/std APIs. For public upstream review, prepare the final patch against the upstream default integration branch, usually `master` or `main`; use installed or stable source trees for reproduction/backport validation, not as the default merge-request base unless the contribution docs explicitly say to target a stable branch. Do not invent a reproducer or user-visible failure that is not in the evidence bundle; if the evidence is profiler-only or indirect, describe it as a targeted mitigation or stop with a diagnosis instead of presenting a speculative patch as a confirmed bug fix. In the plan and final validation, name any such helper, convention, branch-base choice, or evidence limit you found, or say that no relevant local helper was found. Treat this as a `openssh` upstream patch, not just a Debian-local workaround.

Codex subagent/fan-out expectation: for any non-trivial investigation, do not keep all checking in the lead context. Proactively spawn bounded leaf subagents for independent source inspection, duplicate/upstream search, implementation sanity checks, and verification while the lead owns integration. Use the newest available mini model for read-only scouts, and use `gpt-5.3-codex-spark` aggressively for tiny coding, test, and verification leaves even when the lead model is `gpt-5.5`. Pass model names explicitly when spawning. Keep subagents as leaves: tell them not to spawn their own agents, not to run recursive `codex review`, not to perform public/external/destructive actions, and not to duplicate the same live-state operation. Merge their evidence into the final patch/report instead of merely saying a subagent looked. Inspect the relevant code, nearby callers, project contribution docs, and local helper/compat APIs, but do not edit files in this pass.

Return a short markdown plan with these exact sections:

## Problem
## Evidence Confidence
## Proposed Subject
## Patch Plan
## Risks
## Validation

Classify `## Evidence Confidence` as exactly one of `reproduced`, `observed`, or `inferred`. Use `inferred` only for a no-patch diagnosis/report plan unless you can name the extra evidence you will collect before editing; inferred source patches are blocked by Fixer because they are not pull-request-ready. For `observed` source-patch plans, plan to say in the final `## Issue Connection` that the failure was observed by Fixer and not independently reproduced. If the plan touches authentication, authorization, credentials, cryptography, sandboxing, permissions, timing/throttling behavior, or PostgreSQL database-core behavior such as dynamic library resolution, extension loading, transaction semantics, locking, planner/executor behavior, catalog changes, or storage changes, treat it as high-risk: do not plan a source patch unless validation can reproduce the behavior and analyze the semantic impact; otherwise plan a no-patch diagnosis/report. The plan must explain how the proposed code change addresses the observed issue evidence, call out any prior Fixer patch that should be improved or replaced, reject awkward control flow such as avoidable `goto` if there is a cleaner bounded alternative, name any local helper APIs or maintainer conventions the patch should follow, and keep the intended maintainer-facing explanation clear enough that someone unfamiliar with the local complaint wording can still follow the fix. In `## Validation`, name the reproducible configure/build/test entrypoint you will try from the workspace root before any focused leaf compile or smoke check, and include one bounded independent reproduction attempt for the collected failure signal when it is safe and cheap. Do not plan to claim `reproduced` unless that reproduction command or test can actually show the failure.

## Findings
1. Documentation/traceability gap: `/var/lib/fixer/proposals/9382-2026-04-01T22-59-57.796951669+00-00/refine-1-output.txt` describes only a `serverloop.c` EINTR/child-termination fix, but the final workspace patch also changes `[packet.c](/var/lib/fixer/proposals/9382-2026-04-01T22-59-57.796951669+00-00/workspace/packet.c)` (`uncompress_buffer()` no-progress guard). This is actionable because the commit message/Issue Connection and `Git Add Paths` should reflect all shipped functional changes; otherwise reviewability and intent are reduced and downstream cherry-picking risk increases.

Prompt

## Plan Pass

You are planning a fixer patch before any edits happen.

Read the evidence bundle at `./evidence.json`. The prepared workspace is `./workspace` and it was acquired via `debian-source`. The original pre-edit snapshot is available at `./source` if you need to inspect it. Inspect the relevant code, but do not edit files in this pass.

Return a short markdown plan with these exact sections:

## Problem
## Proposed Subject
## Patch Plan
## Risks
## Validation

The plan must explain how the proposed code change addresses the observed issue evidence, call out any prior Fixer patch that should be improved or replaced, and reject awkward control flow such as avoidable `goto` if there is a cleaner bounded alternative.

## Patch Pass

You are working on a bounded fixer proposal.

Read the evidence bundle at `./evidence.json`. The prepared workspace is `./workspace` and it was acquired via `debian-source`. Produce the smallest reasonable patch for the target repository, keep the change upstreamable, prefer the clearest control flow available, and do not keep avoidable `goto` when a simpler structure would read better. The final explanation must connect the observed issue evidence to the actual code change, not just paraphrase the diff.

Start by explaining the likely root cause from the collected perf, strace, and /proc evidence. If you cannot land a safe patch, leave a diagnosis that is strong enough for an upstream bug report.

A previous Fixer patch attempt already exists for this issue. It was generated by Fixer `0.57.0`. Review that patch before changing code, improve it instead of starting blind, and clean up anything awkward or underexplained. In particular, remove avoidable `goto`, tighten the explanation of what the patch is doing, and make the resulting diff feel ready for upstream git review.
- Prior patch: `./prior-best.patch`
- Prior published session: `./prior-best-session.md`

Keep the change narrowly scoped and summarize validation clearly.

In every authoring pass, your final response must start with `Subject: <single-line git commit subject>` and then include these markdown sections exactly:

## Commit Message
A short upstream-friendly explanation of what changed and why.

## Issue Connection
Explain how the code change addresses the observed issue evidence instead of merely paraphrasing the diff.

## Git Add Paths
List the repo-relative paths that belong in the final patch, one per line. Include intentionally new files, and do not list generated build artifacts.

## Validation
List the checks you ran, or say clearly that you could not run them.

Before editing, read the plan at `./plan-output.txt` and follow it unless the code proves part of it wrong. If you change course, say so explicitly in the final write-up instead of silently drifting from the plan.

## Review Pass 1

You are reviewing a freshly generated fixer patch.

Read the evidence bundle at `./evidence.json`. The prepared workspace is `./workspace` and it was acquired via `debian-source`. Review the first patch pass. The original pre-edit snapshot is available at `./source` for diffing. The latest author response is at `./patch-output.txt`. Inspect the current code and changed paths like a strict code reviewer. Focus on correctness, regressions, maintainability, awkward control flow such as avoidable `goto`, missing validation, weak or non-gittable commit message text, and explanations that fail to connect the observed issue evidence to the code change.

Do not apply code changes in this pass.

Return a short markdown review report. The first non-empty line must be exactly one of:

RESULT: ok
RESULT: fix-needed

If you choose `RESULT: fix-needed`, add a `## Findings` section with concrete, actionable items.

## Refinement Pass 1

You are refining a fixer patch after an explicit code review.

Read the evidence bundle at `./evidence.json`. The prepared workspace is `./workspace` and it was acquired via `debian-source`. Read the latest author response at `./patch-output.txt`. Read the review report at `./review-1-output.txt`. This is refinement round 1. The original pre-edit snapshot is available at `./source` if you need to compare the current patch against it. Re-read the planning pass at `./plan-output.txt` before editing. Address the review findings with the smallest reasonable follow-up changes. Keep the patch upstream-friendly, avoid awkward control flow when a simpler structure will do, keep the final response gittable, run relevant tests if available, and summarize which review findings you addressed.

In every authoring pass, your final response must start with `Subject: <single-line git commit subject>` and then include these markdown sections exactly:

## Review Pass 2

You are reviewing a freshly generated fixer patch.

Read the evidence bundle at `./evidence.json`. The prepared workspace is `./workspace` and it was acquired via `debian-source`. Review the patch again after the latest refinement. The original pre-edit snapshot is available at `./source` for diffing. The latest author response is at `./refine-1-output.txt`. Inspect the current code and changed paths like a strict code reviewer. Focus on correctness, regressions, maintainability, awkward control flow such as avoidable `goto`, missing validation, weak or non-gittable commit message text, and explanations that fail to connect the observed issue evidence to the code change.

Response

## Plan Pass

## Problem
The evidence indicates `sshd-session` is consuming CPU in a likely userspace spin: `do_sys_poll` is the hottest kernel symbol and zlib-related symbols in `sshd-session` also appear in top hotspots, with repeated `restart_syscall` and no clear crash path. In current upstream code, two places match this pattern:

- `serverloop.c:wait_until_can_do_something()` does a single `ppoll()` call and returns immediately on `EINTR`, which allows tight outer-loop re-entry under signal churn.
- `packet.c:uncompress_buffer()` runs `inflate(Z_SYNC_FLUSH)` in an unbounded loop; if `inflate` reports `Z_OK` without consuming/providing bytes, the loop can churn without forward progress.

## Proposed Subject
Bound event-wait and decompression loops in `sshd-session` against no-progress spins

## Patch Plan
- Replace the prior-fix approach with a narrower, cleaner version that removes the arbitrary retry behavior and binds retries to elapsed time/progress:
  - File: `serverloop.c`
  - Function: `wait_until_can_do_something(...)`
  - Change plan:
    - Keep a bounded `for` loop around `ppoll()` for `EINTR` only.
    - On each retry, clear `revents` and continue only while timeout state is still valid.
    - When the timeout is exhausted (or no meaningful retry budget remains), return immediately with `conn_in_ready/conn_out_ready = 0`.
    - Do not use `goto`; use explicit condition checks/`break` to keep control flow clear.
- Keep the decompression guard but refine it for correctness and boundedness:
  - File: `packet.c`
  - Function: `uncompress_buffer(...)`
  - Change plan:
    - Snapshot `avail_in`/`avail_out` before each `inflate()` call.
    - On `Z_OK`, compute deltas and append only produced bytes (`progress_out`).
    - If both `progress_in == 0` and `progress_out == 0`, treat as non-advancing decode state and return a controlled error (`SSH_ERR_INTERNAL_ERROR`) after incrementing `compression_in_failures`.
- Call out explicitly: the existing `prior-best.patch` should be replaced by this revised version rather than layered on top, because its polling retry cap is hardcoded and does not tie retry termination to timeout/progress semantics.

## Risks
- The decompression guard may convert a rare benign `Z_OK` no-progress edge into a connection abort; this should be validated with compressed traffic paths and malformed input cases.
- The bounded `ppoll()` retry behavior slightly changes wakeup cadence under heavy interruption; expected to reduce spin but should be checked for any timing-sensitive session behavior.

## Validation
- Static check: confirm both loops now have explicit termination conditions and no new `goto`-based control flow is introduced.
- Runtime checks (planned, not yet run):
  - Build and run OpenSSH unit/integration or at least a focused `make regression` path if available.
  - Reproduce with compressed sessions and signal/interruption pressure; verify no sustained `do_sys_poll`/`restart_syscall` domination.
  - Compare `perf` profiles before/after for reduced user-space spin around `ppoll` and zlib inflate symbols in `sshd-session`.

## Patch Pass

Subject: Guard sshd wait and inflate loops against interrupt/no-progress churn

## Commit Message
Avoid userspace spin in `sshd-session` by (1) handling `ppoll()` interruption retries with real timeout accounting in `wait_until_can_do_something()` and (2) rejecting non-advancing `inflate()` iterations in `uncompress_buffer()`.
This removes the prior fixed retry limit, uses deadline progress to decide when polling should stop retrying, and keeps decompression from looping forever when zlib reports `Z_OK` without consuming input or producing output.

## Issue Connection
The evidence points to a loop pattern with `do_sys_poll` dominating profile time and a `restart_syscall` sample, alongside libz hotspots in `sshd-session`, which is consistent with repeated wakeups that do not make forward progress.
In this codebase, `wait_until_can_do_something()` previously re-entered `ppoll()` on `EINTR` without consuming the caller-visible timeout budget, so a burst of signals could force rapid re-polls and repeated outer-loop cycles while the process stayed effectively spin-ready. The new timeout-deadline recomputation binds retrying `ppoll()` to real elapsed time, so repeated interruptions terminate once the effective timeout is exhausted.
In parallel, `uncompress_buffer()` could call `inflate(Z_SYNC_FLUSH)` and get `Z_OK` while not advancing either input or output; that creates hot-loop behavior in the compressed packet path. The new input/output-progress checks detect that no bytes moved and fail that session’s decompress path deterministically instead of continuing in a no-progress loop.

These two guards directly target the perf/top (`do_sys_poll`) and zlib (`libz`) signatures from the collected evidence rather than applying a superficial change.

## Git Add Paths
serverloop.c
packet.c

## Validation
Could not run tests or build in this pass (not requested).

## Review Pass 1

RESULT: fix-needed

## Findings

- [serverloop.c](./workspace/serverloop.c): Returning on `EINTR` for finite timeouts now changes behavior from “wake and process immediately” to “defer until timeout expires.”
In `wait_until_can_do_something()`, when a timeout is set, the new loop retries `ppoll()` after signal interruptions and only returns when the deadline passes. This can defer `collect_children()` handling of `SIGCHLD` (and session teardown via `session_close_by_pid`) by up to the active timeout (e.g., default rekey interval), which can leave terminated child sessions/processes and deferred exit-status handling longer than expected. Consider keeping immediate return on `EINTR` when `child_terminated` is set, or otherwise ensuring control path breaks out promptly for terminal events.

## Refinement Pass 1

Subject: serverloop: return early from ppoll EINTR when child termination is pending

## Commit Message
The `wait_until_can_something()` EINTR handling now exits immediately when `child_terminated` is set, while preserving the existing timeout-aware retry behavior for ordinary interruptions.

This keeps `sshd` responsive to terminal events (`SIGCHLD`/session teardown path) instead of potentially deferring them until the computed polling timeout expires, while still protecting against userspace spin on noisy signal interruptions.

## Issue Connection
The review finding identified a regression: finite-timeout polling retries on `EINTR` can delay `collect_children()` and session teardown by up to the timeout. The added check directly addresses this by short-circuiting out of the EINTR retry loop when a child exit has been observed, so the caller can immediately service terminal child/session state instead of continuing to burn cycles in another poll-cycle. This preserves the fix against interrupt churn without reintroducing the original unbounded retry behavior.

## Git Add Paths
serverloop.c

## Validation
Could not run tests in this pass.

## Review Pass 2

RESULT: fix-needed

## Findings
1. Documentation/traceability gap: `./refine-1-output.txt` describes only a `serverloop.c` EINTR/child-termination fix, but the final workspace patch also changes `[packet.c](./workspace/packet.c)` (`uncompress_buffer()` no-progress guard). This is actionable because the commit message/Issue Connection and `Git Add Paths` should reflect all shipped functional changes; otherwise reviewability and intent are reduced and downstream cherry-picking risk increases.

## Workflow Note

Review Pass 2 still found unresolved issues after 1 refinement pass(es).

## Problem
Evidence suggests a high-CPU/loop issue in `openssh-server` under `sshd-session` is likely coming from a userspace hot path around polling and/or zlib packet processing:

- Evidence flags `restart_syscall` as dominant and classifies the pattern as `unknown-userspace-loop` (low confidence 0.42).
- Suspicious hotspots resolve to packet/decompression and poll-related code paths (`packet.c` compression flow and `serverloop.c` poll/wait loop).
- Current wait and decode paths can repeatedly re-enter quickly on interruption/retry conditions, which is consistent with restart-driven spin behavior when no useful progress is made.

This is a behavior-level issue, not a single obvious crash bug in packet parsing, and likely needs tighter progress gating and bounded retry behavior.

## Proposed Subject
Avoid unbounded retry/poll-spin loops in `sshd-session` packet wait and decompression paths by enforcing bounded progress and bounded wait/retry behavior in `serverloop.c` and `packet.c`, replacing any fragile retry-only loops that can spin indefinitely after interrupted syscalls.

## Patch Plan
1. Scope update to `serverloop` wait path:
   - File: `./workspace/serverloop.c`
   - Target functions:
     - `wait_until_can_do_something()`
     - `server_loop2()`
   - Add explicit bounded retry/exit conditions around `ppoll()` interruptions so repeated immediate wakeups do not create a tight loop.
   - Preserve protocol behavior by continuing to honor readiness masks, but only loop immediately when there is proof of forward progress (new input/output condition or elapsed timeout).

2. Scope update to packet decompression path:
   - File: `./workspace/packet.c`
   - Target function:
     - `uncompress_buffer()`
   - Add explicit no-progress detection around `inflate()` calls and return a controlled failure code when the inflater cannot advance (instead of re-entering decode retry paths repeatedly on transient conditions).
   - Keep return semantics stable for existing callers and preserve normal success/error status mapping.

3. Optional hardening in monitor read path:
   - File: `./workspace/monitor.c`
   - Target function:
     - `monitor_read()`
   - Normalize poll retry handling so it does not repeatedly churn under `EINTR`/`EAGAIN` without eventual timeout-backed escape.

4. Packaging/debian notes:
   - Verify no existing patch in `debian/patches` targets this exact control-flow issue.
   - If an older fix exists in packaging (or prior candidate patch series) that still relies on fragile retry loops, replace/improve it with this bounded-progress approach in the new patchset.
   - Current installed version history (`1:10.2p1-5` with `-6` available) does not provide a clear user-visible loop fix in top changelog entries, so this should be treated as a source-focused corrective patch rather than a version bump-only replacement.

5. Avoid awkward control-flow constructs:
   - Do not introduce `goto` for loop recovery in new patch logic.
   - Use explicit state checks (`progress made`, `attempts`, `deadline/timeout`) and `for`/`while` loops with clear exit criteria.

## Risks
- A tighter loop boundary may slightly alter wakeup timing under heavy interruption (`EINTR`) and affect micro-performance, though behavior should remain functional equivalent.
- Misclassifying transient zlib state as non-progress could surface higher-level decompression errors sooner than before; must map these errors to existing SSH packet error handling.
- If the true issue is kernel-level scheduling noise rather than userspace logic, impact may be limited and may require complementary tuning after rollout.

## Validation
- Run a focused reproduction with `sshd-session` compression enabled and injected signal/interruption stress to confirm `restart_syscall` no longer dominates.
- Collect fresh `perf top/perf record` after change and verify reduced concentration at `do_sys_poll`/tight userspace polling retry symbols.
- Re-check normal session flows:
  - fresh login, password auth, command exec, interactive shell
  - long-lived idle session with keepalives
  - packet compression on/off
- If available, add/refresh a minimal regression scenario that exercises poll interruption and compressed stream boundaries.

## Patch Pass

Subject: Guard decompression and wait loops against non-progress retry churn

## Commit Message
Fix potential userspace spin in sshd-session by adding bounded retry handling for repeated `ppoll()` interruptions in `serverloop` and explicit no-progress detection in `inflate`-based decompression. This keeps behavior unchanged in normal paths while preventing infinite/quick-retry loops that can dominate CPU without useful progress.

## Issue Connection
The evidence bundle points to two hotspots consistent with this failure mode: `do_sys_poll` is the dominant kernel-side sample, and compressed-path symbols (`libz` inflate/decompress code in `packet.c`) are also hot, with unknown-userspace-loop confidence.
In this codebase, `wait_until_can_do_something()` can return to its caller immediately on every `EINTR`, and its callers can then re-enter the outer session loop quickly. I added bounded `EINTR` retry behavior with an explicit cap so repeated interruption wakeups cannot keep forcing poll-driven churn indefinitely when a timeout is active.

Separately, `uncompress_buffer()` could repeatedly call `inflate(Z_SYNC_FLUSH)` and get no byte-level progress while still returning `Z_OK`, which is exactly the pattern behind a userspace spin in a hot decompression loop: repeated work with no state advancement and no blocking point. The new guard compares `avail_in/avail_out` deltas per inflate iteration and returns a controlled internal error when no input was consumed and no output produced, stopping that non-progress loop and forcing normal packet error handling instead of a busy cycle.

Together this directly addresses the observed “high userspace loop + do_sys_poll + zlib” signature by adding progress gating in both the poll/wait and inflate hot paths.

## Git Add Paths
/serverloop.c
/packet.c

## Validation
- Checks run: none (no build/tests run per request).
- Changes were kept minimal and scoped to `serverloop.c` and `packet.c` only.

## Review Pass 1

RESULT: ok

## Findings
No concrete correctness, regression, or maintainability issues were found in this first-pass patch set. The changes in `serverloop.c` and `packet.c` are internally consistent with the described issue and do not introduce obvious unsafe control-flow or validation regressions from a static review.
